Продемонстрировать опыт эндоваскулярного лечения пациентов с тромбоэмболией верхней брыжеечной артерии (ВБА).</p></sec><sec><title>МЕТОДЫ И МАТЕРИАЛЫ</title><p>МЕТОДЫ И МАТЕРИАЛЫ. В клинике эндоваскулярному вмешательству подверглись 13 пациентов с тромбоэмболией ВБА. Мужчин было 4 (31 %), женщин – 9 (69 %). Средний возраст составил (76±9) лет. Сроки от момента начала заболевания до операции и реперфузии верхней брыжеечной артерии колебались от 4 до 65 ч, в среднем составляя (22±17) ч. Исходный лейкоцитоз у пациентов составил (16,5±5,8)·10/9 л (от 9,2 до 28,8·10/9 л). Применяли различные эндоваскулярные манипуляции для реперфузии артерии – механическую реканализацию, баллонную ангиопластику, аспирационную тромбэктомию и стентирование артерии.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Полное восстановление магистрального антеградного кровотока по ВБА и ее основным ветвям достигнуто у всех 13 (100 %) пациентов. Лапаротомия выполнена у 4 (31 %) больных, при этом некроз кишечника после эндоваскулярного вмешательства развился в 3 (23 %) случаях. Летальность составила 46 %. Основной причиной летального исхода явился реперфузионный синдром (4 пациента, 31 %).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Эндоваскулярные методы лечения тромбоэмболии ВБА можно рассматривать в качестве первого шага в лечебном алгоритме. Они позволяют у большинства пациентов сохранить значительную часть жизнеспособного кишечника. Реперфузионный синдром явился основной причиной летальных исходов. Разработка методов профилактики и алгоритма лечения реперфузионного синдрома при острой окклюзии ВБА должна явиться основой успеха в лечении этих пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><p>THE OBJECTIVE was to report our experience of endovascular treatment in patients with acute thromboembolic occlusion of the superior mesenteric artery (SMA).</p><sec><title>METHODS AND MATERIALS</title><p>METHODS AND MATERIALS. 13 patients with acute thromboembolic occlusion of the SMA underwent endovascular intervention. There were 4 (31 %) men and 9 (69 %) women. The average age was 76±9 years. The time from the beginning of the disorder to intervention and mechanical reperfusion of the SMA ranged from 4 to 65 hours, averaging (22±17) hours. Initial leukocytosis was (16.5±5.8)·10/9 l (from 9.2 to 28.8·10/9 l). Various endovascular manipulations were applied to achieve artery reperfusion: mechanical recanalization, balloon angioplasty, aspiration thrombectomy and stenting.</p></sec><sec><title>RESULTS</title><p>RESULTS. Complete recovery of antegrade blood flow through the SMA and its main branches was achieved at all 13 (100 %) patients. Laparotomy was executed at 4 (31 %) patients, thus the intestinal necrosis after endovascular intervention developed in 3 (23 %) cases. The lethal outcome was 46 %. The main reason for a lethal outcome was reperfusion syndrome (4 patients, 31 %).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Endovascular methods of treatment of patients with acute thromboembolic occlusion of the SMA can be considered as the first step in medical algorithm. They allow to save intestinal viability in most patients. Reperfusion syndrome was the main reason for a lethal outcome.
