Оценка среднесрочных результатов различных видов дебранчинга в гибридной хирургии дуги и нисходящей грудной аорты.</p></sec><sec><title>МЕТОДЫ И МАТЕРИАЛЫ</title><p>МЕТОДЫ И МАТЕРИАЛЫ. Оперированы 47 пациентов с различными патологиями дуги и нисходящей грудной аорты. Из них 6 с расслоением типа IIIa и 26 с расслоением типа IIIb по классификации DeBakey, 15 больных - с истинными аневризмами аорты. Выполнены следующие вмешательства: тотальный дебранчинг дуги аорты (n=5), перекрестное подключично-подключично-левообщесонное (n=14) или перекрестное сонно-сонно-подключичное шунтирование (n=7); подключично-сонная транспозиция или шунтирование (n=12); техника «чимни» при аневризме дуги аорты (n=8); фенестрирование эндопротеза in situ (n=1). Плановое вмешательство выполнено у 32 пациентов, экстренное - у 15.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Технический успех достигнут в 100 % случаев. Общая госпитальная летальность составила 10,6 % (n=5), после плановой операции - 9,3 % (n=3), после экстренной - 13,3 % (n=2). Причины летальности: острое нарушение мозгового кровообращения (n=1), синдром полиорганной недостаточности после протезирования восходящей аорты из-за ретроградного расслоения после имплантации стент-графта (n=1), пневмония (n=1), острый инфаркт миокарда (n=1), профузное кровотечение (n=1). Кумулятивная выживаемость составила 87,5 % в течение 5 лет, свобода от реинтервенций - 89,1 %. В течение 1-го года 3 пациента перенесли повторное эндопротезирование нисходящей грудной аорты при формировании расслоения ниже стент-графта (n=1), протезирование восходящей аорты при ее ретроградном расслоении (n=1), эндофиксацию стент-графта с эмболизацией ложного канала (n=1).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Гибридные операции на дуге и нисходящей грудной аорте являются относительно безопасным и эффективным методом лечения. Регулярное наблюдение и своевременное лечение осложнений в отдаленные сроки после гибридного или эндоваскулярного лечения являются обязательными для улучшения отдаленных результатов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>OBJECTIVE</title><p>OBJECTIVE. Assessment of the mid-term results after various types of debranching in hybrid surgery of the aortic arch and descending thoracic aorta.</p></sec><sec><title>METHODS AND MATERIALS</title><p>METHODS AND MATERIALS. 47 patients with various pathologies of the aortic arch and descending thoracic aorta were operated on. Six of them suffered from type IIIa dissection and 26 from type IIIb dissection according to DeBakey classification, 15 patients had true aortic aneurysms. The following interventions were performed: total aortic arch debranching (n=5), cross subclavian-subclavian-left-common carotid (n=14) or cross carotid-carotid-subclavian bypass (n=7); subclavian-carotid transposition or bypass (n=12); Chimney technique for the aortic arch aneurysm (n=8); in situ fenestration of the endoprosthesis (n=1). Elective surgery was performed in 32 patients, emergency in 15.</p></sec><sec><title>RESULTS</title><p>RESULTS. Technical success achieved in 100 %. The overall hospital mortality rate was 10.6 % (n=5), elective surgery mortality rate was 9.3 % (n=3), in emergency cases - 13.3 % (n=2). Causes of mortality: stroke (n=1), multiple organ failure after prosthetic ascending aorta due to retrograde dissection after stent graft implantation (n=1), pneumonia (n=1), acute myocardial infarction (n=1), profuse bleeding (n=1). Cumulative survival rate was 87.5 % over 5 years, freedom from reinterventions - 89.1 %. During the first year of observation, 3 patients underwent stent graft replacement of the descending thoracic aorta due to formation of a dissection below the primary stent graft (n=1), open prosthesis of the ascending aorta due to it retrograde dissection (n=1), endofixation of the stent graft with embolization of the false lumen (n=1).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Hybrid surgery on the aortic arch and the descending thoracic aorta is safe and effective treatment.
