Провести анализ результатов эндоскопического транссфеноидального удаления опухоли хиазмально-селлярной области у детей.</p></sec><sec><title>МЕТОДЫ И МАТЕРИАЛЫ</title><p>МЕТОДЫ И МАТЕРИАЛЫ. Исследованы данные 23 пациентов в возрасте от 3 до 17 лет, которым было выполнено эндоскопическое транссфеноидальное удаление объемных образований хиазмально-селлярной области. Проведена оценка неврологических, офтальмологических, эндокринных нарушений и оториноларингологической патологии. Исследовали нейровизуализационные данные (магнитно-резонансная томография и компьютерная томография), результаты лабораторных анализов. Дополнительно выполняли анализ хирургического лечения, в том числе интраоперационных и ранних послеоперационных осложнений.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В 39,1 % случаев диагностированы новообразования супрапараинфраселлярной локализации. У 34,8 % больных размер объемных образований не превышал 2 см. У 1 пациента объем неоплазмы составлял 53,7 см3. По типу пневматизации преобладал селлярный тип. Раковинный тип наблюдался у 1 ребенка. Чаще (86,9 %) использовали гемостатический материал «Surgicel Fibrillar». Пластику дефекта черепа осуществляли поэтажно пластинами «ТахоКомба» (62,5 %). Тотальное удаление опухоли достигнуто у 75 % детей. Большинство опухолей было представлено краниофарингиомой – 30,43 %. В раннем послеоперационном периоде отмечался значительный регресс неврологических нарушений – на 69,6 %, а также офтальмологических – на 34,8 % и эндокринных – на 37,8 %. Случаев назальной ликвореи и эпистаксиса у детей не наблюдалось.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Наше исследование продемонстрировало успешность удаления новообразований у детей от 3 лет, независимо от размера полости носа и пневматизации клиновидной пазухи. Возможно использование у детей эндоскопического транссфеноидального доступа для удаления объемных образований хиазмально-селлярной области более 5 см, так как минимизирован риск интра- и послеоперационных осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><p>The OBJECTIVE was to analyze the results of endoscopic transsphenoidal removal of neoplasms of the chiasmalsellar region in children.</p><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. We studied 23 patients aged 3 to 17 years who underwent endoscopic transsphenoidal removal of neoplasms of the chiasmal-sellar region. Neurological, ophthalmological, endocrine disorders and otorhinolaryngological pathology were evaluated. Neuroimaging data (MRI and CT), results of laboratory tests were studied. In addition, the analysis of surgical treatment including intraoperative and early postoperative complications was performed.</p></sec><sec><title>RESULTS</title><p>RESULTS. In 39.1 % of cases, neoplasms of supraparainfracellar localization were diagnosed. In 34.8 % of patients, the size of the neoplasms did not exceed 2 cm. In 1 patient, the volume of neoplasm was 53.7 cm3. According to the type of pneumatization, the sellar type prevailed. Сonch type was observed in 1 child. Hemostatic material «Surgicel Fibrillar» was used more often (86.9 %). Plastic surgery of the skull defect was carried out on a floor-by-floor basis with «Tachocomb» plates (62.5 %). Total tumor removal was achieved in 75 % of children. The majority of neoplasms were represented by craniopharyngioma – 30.43 %. In the early postoperative period, there was a significant regression of neurological disorders – by 69.6 %, as well as ophthalmological – by 34.8 % and endocrine – by 37.8 %. There were no cases of nasal liquorrhea and epistaxis in children.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Our study demonstrated the success of removing neoplasms in children from 3 years old, regardless of the size of the nasal cavity and pneumatization of the sphenoid sinus.
