Проанализировать собственный опыт выполнения тиреоидэктомии (ТЭ) у больных амиодарон-индуцированным тиреотоксикозом (АмИТ).</p></sec><sec><title>МЕТОДЫ И МАТЕРИАЛЫ</title><p>МЕТОДЫ И МАТЕРИАЛЫ. В исследование включили 12 пациентов с АмИТ, которым была выполнена ТЭ. Были проанализированы терапевтические истории болезней для оценки особенностей течения АмИТ и показаний к ТЭ. Кроме этого, были изучены протоколы оперативного вмешательства и данные послеоперационного наблюдения. Оценивали интраоперационные, ранние и отдаленные послеоперационные осложнения. Отдаленные результаты ТЭ оценивали с использованием эхокардиографии по динамике фракции выброса левого желудочка (ФВ ЛЖ).</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Основными показаниями к ТЭ были резистентность тиреотоксикоза к медикаментозной терапии и ухудшение течения кардиальной патологии. Ни у одного пациента во время операции не отмечалось нарастания явлений тиреотоксикоза или развития тиреотоксического криза. У 1 больного развился парез голосовой связки с восстановлением ее подвижности через год. Кровопотеря во всех случаях была минимальной. В раннем послеоперационном периоде летальных исходов не было. У 1 пациентки с аритмогенной дисплазией правого желудочка наблюдался короткий пароксизм фибрилляции предсердий, который купировался самостоятельно. Через 39 дней после операции умер 1 пациент, причиной смерти стала массивная тромбоэмболия ветвей легочной артерии на фоне бивентрикулярной хронической сердечной недостаточности высокого функционального класса. Отдаленные результаты ТЭ были оценены у 8 пациентов. У 4 из 5 больных со сниженной исходно ФВ ЛЖ она увеличилась. У 3 пациентов с нормальной на момент ТЭ ФВ ЛЖ она не изменилась.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Тиреоидэктомия является эффективным и безопасным методом лечения АмИТ, включая пациентов с персистирующим тиреотоксикозом и тяжелой кардиальной патологией. Подготовка больных к операции и выполнение данного вмешательства должны осуществляться командой специалистов, обладающих опытом лечения таких пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><p>The OBJECTIVE of the study was to analyze the experience of performing thyroidectomy (TE) in patients with amiodarone-induced thyrotoxicosis (AmIT) at our centre.</p><sec><title>METHODS AND MATERIALS</title><p>METHODS AND MATERIALS. The study included 12 patients with AmIT who underwent TE. Medical records were analyzed to assess the features of the AmIT and indications for TE. We also studied the operation protocols and postoperative follow-up data. Intraoperative, early and long-term postoperative complications were recorded. The long-term TE results were evaluated by the dynamics of the left ventricular ejection fraction (LVEF) based on the echocardiography data.</p></sec><sec><title>RESULTS</title><p>RESULTS. The main indications for TE included the resistance of thyrotoxicosis to medication and worsening of the cardiac pathology. No cases of thyrotoxicosis progression or thyrotoxic crisis were registered during the operation. The vocal cord paresis developed in one case, completely restored in a year. Blood loss was minimal in all cases. Other intraoperative complications were absent. Not a single death was registered in the early postoperative period. At this period, a short paroxysm of atrial fibrillation resolved on its own was registered in patient with arrhythmogenic right ventricular dysplasia. A patient with biventricular chronic heart failure of a high functional class died 39 days after the operation due to a massive pulmonary thromboembolism. The long-term results of TE were evaluated in eight patients. In four out of five patients with initially reduced LVEF, it increased. In three patients with initially normal LVEF, it did not change.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Thyroidectomy is an effective and safe treatment in patients with AmIT, including those with the persistent thyrotoxicosis and severe cardiac pathology.
