На основании собственного опыта определить частоту, структуру и специфику отдаленных осложнений после замещения дефектов мягких тканей и костей голени, разработать тактику лечения.</p></sec><sec><title>МЕТОДЫ И МАТЕРИАЛЫ</title><p>МЕТОДЫ И МАТЕРИАЛЫ. Проведено ретроспективное исследование историй болезни 53 пациентов, проходивших лечение в травматологическом отделении госпиталя за период 2000 по 2008 г. Данные пациенты имели раны мягких тканей и циркулярные дефекты диафиза большеберцовой кости в результате тяжелых открытых повреждений (17 (32,1 %) пострадавших) и огнестрельных ранений голени (36, 67,9 %). Пациентам проводили замещение дефектов мягких тканей и костей голени методом Илизарова и микрохирургическими пересадками кровоснабжаемых лоскутов. Проведен анализ результатов лечения.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. После окончания стационарного лечения проводили динамическое наблюдение. За медицинской помощью обратились 39 (73,6 %) из 53 пациентов, в том числе по поводу различных осложнений, средний срок долгосрочного наблюдения составил (42,2±5,5) месяца. Наиболее частыми осложнениями были рецидивы воспаления мягких тканей - 13,7 % и остеомиелит - 24, 5 % случаев, из невоспалительных осложнений имели место переломы регенерата (5,7 %) и ложные суставы (7,6 %). Приведен анализ методов лечения данных осложнений.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Одномоментное замещение обширных дефектов голени свободным васкуляризированным лоскутом и дистракционным остеогенезом по Илизарову создает благоприятные условия для заживления раны и восстановления большеберцовой кости, однако в отдаленном периоде имеют место осложнения в виде рецидивов остеомиелита, переломов регенерата и ложных суставов, которые требуют специализированного лечения, основанного на детальном анализе патологии и предшествующей методики лечения.</p></sec></abstract><trans-abstract xml:lang="en"><p>The OBJECTIVE of the study, based on our own experience, was to determine the frequency, structure and specificity of long-term complications after replacement of soft tissue and leg bones defects, and to develop treatment tactics.</p><sec><title>METHODS AND MATERIALS</title><p>METHODS AND MATERIALS. A retrospective study of the case of 53 patients who underwent treatment in the trauma department of the hospital for the period 2000 to 2008 was carried out. These patients had soft tissue wounds and circular defects of the tibial diaphysis because of severe open injuries of 17 (32.1 %) and gunshot wounds to the lower leg (36 (67.9 %). Patients underwent replacement of soft tissue and lower leg bones using the Ilizarov method and microsurgical reconstruction free flaps. The analysis of treatment results was carried out.</p></sec><sec><title>RESULTS</title><p>RESULTS. After the end of inpatient treatment, patients were dynamic observation carried out. 39 (73.6 %) of 53 patients sought medical help, as for various complications, the average long-term follow-up was (42.2±5.5) months. The most frequent complications were relapses of soft tissue inflammation in 13.7 % and osteomyelitis in 24.5 % of cases, non-inflammatory complications were tibia fractures (5.7 %) and pseudarthrosis (7.6 %). The analysis of treatment methods of these complications is presented.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Simultaneous replacement of extensive leg defects with a free vascularized flap and Ilizarov's distraction osteogenesis creates favorable conditions for wound healing and tibial restoration, however, in the long-term period there are complications in the form of recurrent osteomyelitis, tibia fractures and pseudarthrosis, which require specialized treatment based on a detailed analysis of the pathology and the previous treatment method.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>дефект мягких тканей голени</kwd><kwd>дефект большеберцовой кости</kwd></kwd-group><kwd-group xml:lang="en"><kwd>soft tissue defect of the lower leg</kwd><kwd>tibial defect</kwd><kwd>long-term consequences of lower leg injuries</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Новый подход к профилактике гнойно-некротических и септических осложнений при остеосинтезе переломов дистального отдела голени / И.
