Выявление предикторов дисфункции биологических протезов аортального клапана (АО) в отдаленном периоде после хирургического лечения.</p></sec><sec><title>МЕТОДЫ И МАТЕРИАЛЫ</title><p>МЕТОДЫ И МАТЕРИАЛЫ. Проанализированы законченные случаи лечения за период с 2015 по 2020 г. и данные эхокардиографии 74 пациентов (42 мужчин и 32 женщин) (средний возраст - (67,0±4,2) года) с имплантированными в аортальную позицию биопротезами. Средний срок наблюдения составлял 4 года. Оценивали скорость трансаортального потока, средний и пиковый трансклапанный градиенты, площадь эффективного отверстия протеза, индекс массы миокарда, толщину и подвижность створочного аппарата биоклапана. Наличие у пациента выраженного протез-пациентного несоответствия (ППН) устанавливали при значениях iЕОА менее 0,65 см2/м2, умеренного - при значениях от 0,65 до 0,85 см2/м2, легкого - до 0,85 см2/м2.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. ППН было зафиксировано в 27 % случаев. Структурная дисфункция наблюдалась в 12,5 % случаев, она включала в себя преимущественно стенозирование биопротеза (10 %). В 2,5 % случаев была выполнена реоперация по поводу тяжелого стеноза биопротеза АК. В ходе многофакторного анализа было выявлено, что ППН явилось предиктором развития структурной дисфункции биопротезов аортального клапана в отдаленном периоде после операции (р&lt;0,005).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>OBJECTIVE</title><p>OBJECTIVE. Identification of predictors of dysfunction of biological aortic valve prostheses in the long-term period after surgical treatment.</p></sec><sec><title>METHODS AND MATERIALS</title><p>METHODS AND MATERIALS. We analyzed the completed treatment cases for the period from 2015 to 2020 and echocardiography data of 74 patients (42 men and 32 women) with an average age of 67.0±4.2 years with bioprostheses implanted in the aortic position. The average follow-up period was 4 years. The transaortic flow velocity, the average and peak transvalvular gradients, the area of the effective opening of the prosthesis, the myocardium mass index, the thickness and mobility of the cusp of the biovalve were evaluated. The presence of the pronounced PPM in a patient was determined at iEOA values less than 0.65 cm2/m2, moderate - at values from 0.65 to 0.85 cm2/m2, mild - up to 0.85 cm2/m2.</p></sec><sec><title>RESULTS</title><p>RESULTS. PPM was recorded in 27% of cases. Structural dysfunction was observed in 12.5% of cases, it mainly included stenosis of the bioprosthesis (10%). In 2.5% of cases, reintervention was performed for severe stenosis of the AV bioprosthesis.
