С 01.01.2016 г. по 31.12.2022 г. в нашей клинике было оперировано 69 больных раком пищевода I–III стадии, которым была выполнена эзофагэктомия по McKeown в полностью открытом варианте (торакотомия, лапаротомия, цервикотомия). Была проведена оценка характеристик исследуемых пациентов по единому разработанному протоколу. Определены статистически значимые факторы, влияющие на развитие несостоятельности анастомоза.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Частота несостоятельности анастомоза составила 13/69 (18,4 %). Умерло от несостоятельности анастомоза 3 (23 %) пациента, в группе без несостоятельности анастомоза умерло 4 (7,1 %) пациента. Средний послеоперационный койко-день составил 11,57±3,71 дня в группе без несостоятельности анастомоза, и 25,42±8,12 в группе несостоятельности анастомоза (p=0,0 01). Одномерный анализ показал, что статистически значимо влияют на несостоятельность наличие сахарного диабета, гемотрансфузии в послеоперационном периоде, а также наличие ХБП больше 3 степени. Эти факторы были выбраны для множественного логистического регрессионного анализа. В многомерной логистической регрессии только ХБП и СД были подтверждены как значимые факторы риска несостоятельности анастомоза. Несостоятельность III и IV типа возникла у 5/13 (38,5 %) пациентов, из них умерло 3 (23 %).</p></sec><sec><title>ВЫВОДЫ</title><p>ВЫВОДЫ. Статистически значимыми факторами риска развития несостоятельности анастомоза после эзофагэктомии по McKeown при однофакторном анализе стали: СД, ХБП, и периоперационная гемотрансфузия до развития несостоятельности. Неудовлетворительные результаты лечения пациентов после эзофагэктомии возникают при III и IV типе несостоятельности.</p></sec></abstract><trans-abstract xml:lang="en"><p>The OBJECTIVE was to identify risk factors for anastomotic leakage following McKeown esophagectomy.</p><sec><title>METHODS AND MATERIALS</title><p>METHODS AND MATERIALS. From January 1, 2016 to December 31, 2022, in our clinic, a total of 69 patients with stage I–III esophageal cancer were operated with McKeown esophagectomy performed in a fully open approach (thoracotomy, laparotomy, cervicotomy). The characteristics of the studied patients were evaluated according to a standardized protocol. Statistically significant factors influencing the development of anastomotic failure were determined.</p></sec><sec><title>RESULTS</title><p>RESULTS. The frequency of anastomotic leakage was 13/69 (18.4 %). Mortality due to anastomotic failure occurred in 3 (23 %) patients; while in the non-failure group, there were 4 (7.1 %) fatalities. The average postoperative hospital stay was 11.57±3.71 days in the non-leakage group and increased to 25.42±8.12 days in the leakage group (p=0.001). Univariate analysis revealed that the presence of diabetes mellitus, postoperative blood transfusions, and higher than Grade III chronic kidney disease significantly influenced anastomotic integrity. These factors were selected for multiple logistic regression analysis. In multivariable logistic regression only chronic kidney disease and diabetes mellitus were confirmed as significant risk factors for anastomotic failure; Type III and IV failures occurred in 5/13 (38.5 %) patients resulting in death for three individuals (23 %).</p></sec><sec><title>CONCLUSIONS</title><p>CONCLUSIONS. In univariate analysis, diabetes mellitus, chronic kidney disease and perioperative blood transfusion were recognized as statistically significant risk factors associated with anastomotic leakage following McKeown esophagectomy.
