С марта 2019 г. по сентябрь 2023 г. на отделении сосудистой хирургии ПСПбГМУ им. И. П. Павлова были оперированы 55 пациентов с аневризмами висцеральных артерий. У 4 больных аневризмы почечной артерии были оперированы открытым хирургическим способом, у 5 пациентов была выполнена эндоваскулярная операция. Приведены два клинических случая открытой реконструкции аневризмы почечной артерии in situ и один ex vivo. Во всех случаях показанием к операции было наличие симптомной аневризмы. Для реконструкций in situ были использованы забрюшинный торакофренопараректальный доступ по десятому межреберью слева с вскрытием левой плевральной полости и частичной диафрагмотомией и поперечная лапаротомия типа «chevron». В качестве шунта применялась аутовена. Для пластики почечной артерии ex vivo выполнена лапароскопическая нефрэктомия с аутотрансплантацией почки. В ближайшем послеоперационном периоде проведена оценка проходимости зоны реконструкции МСКТ ангиографией, анализ показателей почечной функции.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Во всех наблюдениях отмечены удовлетворительные ближайшие результаты – зона реконструкции проходима, показатели почечной функции без отрицательной динамики, отсутствие клинических проявлений данной патологии. Отдаленные результаты (наблюдение более 4 лет) прослежены у одного больного. На протяжении этого срока сохраняется нормальная почечная функция и адекватное функционирование зоны реконструкции.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Открытые реконструктивные операции в хирургическом лечении аневризм почечной артерии сохраняют свою актуальность в связи с невозможностью в ряде случаев провести ее эндоваскулярную коррекцию без высокого риска ишемического повреждения паренхимы почки.</p></sec></abstract><trans-abstract xml:lang="en"><p>THE OBJECTIVE was to evaluate the immediate results of open reconstructive operations for renal artery aneurysm.</p><sec><title>METHODS AND MATERIALS</title><p>METHODS AND MATERIALS . From March 2019 to September 2023, 55 patients with visceral artery aneurysms were operated at the Department of Vascular Surgery at the Pavlov University. 4 patients with renal artery aneurysms were operated on by open surgical method and 5 patients underwent endovascular operation. There are two clinical cases of open reconstruction of renal artery aneurysm in situ and one ex vivo. In all cases, the indication for the operation was the presence of a symptomatic aneurysm. For in situ reconstructions, a retroperitoneal thoracophrenopararectal access to the tenth intercostal space on the left side with the opening of the left pleural cavity and partial diaphragmatic cavity and a transverse laparotomy of «chevron» type were used. Autovena was used as a bypass. For surgery of the renal artery ex vivo laparoscopic nephrectomy with autotransplantation of the kidney was performed. In the near postoperative period, we made the assessment of the patency of the reconstruction zone using MSCT angiography, the analysis of indicators of renal function.</p></sec><sec><title>RESULTS</title><p>RESULTS . All the observations noted satisfactory near-term results – the reconstruction zone is unobstructed, indicators of renal function without negative dynamics, absence of clinical manifestations of this pathology. Long-term results   (observed for more than 4 years) were traced in one patient. During this period, normal renal function and adequate functioning of the reconstruction zone are maintained.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION .
