Одной из важных проблем в сердечно-сосудистой хирургии является наличие многоуровневых поражений артерий нижних конечностей. Нами продемонстрированы результаты вмешательств при многоуровневых поражениях артерий нижних конечностей.</p></sec><sec><title>МЕТОДЫ И МАТЕРИАЛЫ</title><p>МЕТОДЫ И МАТЕРИАЛЫ. Проанализированы результаты лечения 90 пациентов с сочетанным поражением аорто-бедренного и бедренно-подколенного сегмента типа TASC C и D. Пациенты разделены на 2 группы и 2 подгруппы. Группа 1 – пациенты, которым выполнены комбинированные многоуровневые вмешательства, в свою очередь, группа 1 разделена на 2 подгруппы по типу выполненной реконструкции. Группа 1А – закрытая петлевая эндартерэктомия из подвздошных артерий + бедренно-подколенное (тибиальное) шунтирование. Группа 1Б – аорто (подвздошно) – бедренное шунтирование + петлевая эндартерэктомия из артерий бедренно-подколенного сегмента. Группа 2 – пациенты, которым сочетанно выполнено аорто (подвздошно)-бедренное + бедренно-подколенно (тибиальное) шунтирование.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В группе 1 зарегистрированных случаев тромбоза шунта в раннем послеоперационном периоде не было, тогда как в группе 2 в трех случаях (12 %) происходил тромбоз как аорто-бедренного, так и бедренно-подколенного шунта в раннем послеоперационном периоде. В группе 1 зарегистрирован 1 эпизод кровотечения, потребовавший ревизию, в группе 2 кровотечений зарегистрировано не было. В 1 и во 2 группе зарегистрировано по 1 случаю инфаркта миокарда (ИМ) в раннем послеоперационном периоде (p=0,301), у 1 больного в группе 2 произошел инсульт с регрессом симптоматики в послеоперационном периоде. В группе 1 смертей и ампутаций в раннем послеоперационном периоде зарегистрировано не было. В группе 2 двое больных (8 %) перенесли ампутацию в раннем послеоперационном периоде, один больной скончался в послеоперационном периоде. Первичная проходимость в группе 1А к 12, 24 и 60 месяцу составила 95 %, 76 % и 63 % соответственно, в группе 1Б в те же сроки месяцу составила 86 %, 86 % и 81 % соответственно, в группе 2 в те же сроки 100 %, 100 % и 85 % соответственно (p=0,368). Вторичная проходимость в группе 1А к 12 месяцам – 77 %, к 24 месяцам 77 % и к 60 месяцам 52 %. В группе 1Б – к 12 месяцам – 100 %, к 24 месяцам 67 %. В группе 2 к 12 месяцу и 24 месяцу 50 % (p=0,983).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Таким образом, при невозможности выполнения гибридных процедур комбинированное вмешательство при многоуровневых протяженных поражений артерий нижних конечностей является приемлемой альтернативой многоэтажным шунтированиям. Разницы в отдаленной проходимости между сравнивамемыми методами вмешательств не было, однако наличие фатальных осложнений в группе многоуровневых шунтирований говорит о преимуществе менее инвазивного подхода.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION. One of the important problems in cardiovascular surgery is the multilevel lesions of the lower extremity arteries. We presented the results of treatment of multilevel arterial lesion.</p></sec><sec><title>METHODS AND MATERIALS</title><p>METHODS AND MATERIALS. We analyzed the treatment results of 90 patients with combined lesions of the aortofemoral and femoral-popliteal segment of type TASC C and D. The patients were divided into 2 groups and 2 subgroups. Group 1 consisted of patients who underwent combined multilevel interventions, in turn, group 1 was divided into 2 subgroups according to the type of interventions. Group 1A – closed remote endarterectomy (RE) from the iliac arteries + femoral-popliteal (tibial) bypass surgery. Group 1B – aorto (iliac) – femoral bypass + RE from the arteries of the femoral-popliteal segment. Group 2 consisted of patients who underwent combined aorto (iliac) femoral + femoral-popliteal (tibial) bypass surgery.</p></sec><sec><title>RESULTS</title><p>RESULTS. There were no registered cases of shunt thrombosis in the early postoperative period in group 1, whereas in group 2, both aorto-femoral and femoral-popliteal shunt thrombosis occurred in three cases (12%) in the early postoperative period. In group 1, 1 episode of bleeding was registered that required revision, in group 2, no bleeding was registered. In groups 1 and 2, 1 case of myocardial infarction (MI) was registered in the early postoperative period (p=0.301), one patient in group 2 had a stroke with regression of symptoms in the postoperative period. There were no deaths or amputations in the early postoperative period in group 1. In group 2, two patients (8%) underwent amputation in the early postoperative period, one patient died in the postoperative period. The primary patency in group 1A by 12, 24 and 60 months was 95 %, 76 % and 63 % respectively, in group 1B by 12, 24 and 60 months was 86 %, 86 % and 81 %, respectively, in group 2 by 12, 24 and 60 months was 100 %, 100 % and 85 %, respectively (p=0.368). Secondary patency in group 1A by 12 months was 77 %, by 24 months was 77 % and by 60 months was 52 %. In group 1B – by 12 months – 100 %, by 24 months – 67 %. In group 2 – by 12 months and 24 months – 50 % (p =0.983).</p></sec><sec><title>CONCLUSIONS</title><p>CONCLUSIONS. Thus, if it is impossible to perform hybrid procedures, combined interventions for multi-level lesions of lower extremity arteries is an acceptable alternative to multi-level bypass.
