Определить роль билиарного манометрического теста (БМТ) в оценке эффективности чрескожной баллонной пластики доброкачественных стриктур желчных протоков после ортотопической трансплантации печении (ОТП) и других оперативных вмешательств.МЕТОДЫ И МАТЕРИАЛЫ. В период 2009–2025 гг. БМТ использован у 37 больных. У 15 пациентов неанастомотические стриктуры развились через 4–106 (в среднем 17) месяцев после ОТП; у 22 отмечены стриктуры через 2–139 (в среднем19) месяцев после операций на желчных путях. После чрескожного холангиодренирования осуществляли баллонные дилатации до исчезновения стриктуры. При достижении положительного рентгенологического результата выполняли БМТ. РЕЗУЛЬТАТЫ. У 37 больных осуществили 180 процедур (от 2 до 17, в среднем 6). БМТ выполняли в среднем через 10 (6–17) месяцев.У 4 пациентов лечение продолжается. Технический успех достигнут у 30 из 33 (90 %) и у них были удалены холангиодренажи в сроки от 8 до 22 (в среднем 11) месяцев. Рецидив стриктуры возник у двух, а еще у трех пациентов лечение было неэффективным; во всех 5 наблюдениях выполнена хирургическая реконструкция желчных протоков. Таким образом, клинический успех достигнут у 28 из 33 больных (85 %). Сроки наблюдения составляют от 12 до 172 (в среднем 51) месяцев, без признаков билиарной гипертензии.После удаления дренажа на основании БМТ проходимость желчных протоков в течение года была 100 %, в течение 4 лет 93 %.ЗАКЛЮЧЕНИЕ. БМТ представляется объективным методом контроля эффективности баллонной пластики послеоперационных доброкачественных стриктур желчных протоков. Проходимость сохраняется у 93 % пациентов в течение 4-х лет при положительном БМТ.</p></abstract><trans-abstract xml:lang="en"><p>METHODS AND MATERIALS. Between 2009 and 2025, BMT was used in 37 patients. In 15 patients, non-anastomotic strictures developed 4-106 (on average 17) months after OLT; in 22 patients, strictures were noted after 2-139 (on average 19) months after biliary surgery. After percutaneous biliary drainage, balloon dilatations were performed until disappearance of stricture. Upon achieving a positive X-ray result, BMT was performed.RESULTS. Patients (n=37) received 180 (2–17, mean 6) procedures. BMT was performed after on average 10 (6–17) months. Treatment continues in 4 patients. Technical success was achieved in 30 of 33 patients (90 %), and their biliary drainage was removed in 8–22 (mean 11) months. Stricture recurrence developed in 2, also treatment was ineffective in 3; all 5 these patients underwent successful surgical biliary reconstruction. Thus, clinical success was achieved in 28 of 33 patients (85 %). Follow-up periods range from 12 to 172 (on average 51) months, with no signs of biliary hypertension. Adequate 1-yr and 4-yr biliary decompression was 100 % and 93 % after removal of the drainage on the basis of BMT.CONCLUSION. BMT seems to be objective method to control effectiveness of balloon cholangioplasty for postoperative benign biliary strictures.
